Robert was admitted to TriStar Hendersonville Medical Center following a recent decline in functional status. Prior to admission, Mr. Ramsey experienced increasing episodes of generalized weakness accompanied by multiple falls at home, raising concerns for his safety and ability to perform activities of daily living independently.

Due to the progression of his symptoms and heightened fall risk, he was referred for skilled nursing care and physical therapy services aimed at improving strength, mobility, and overall functional independence.

Robert was admitted to Gallatin Center for Rehabilitation and Healing on January 29, 2026. Upon admission, Robert was introduced to the concierge, his therapy team, nursing team, respiratory therapist, social services, dietary director, and activities director, and a plan of care was created specifically for his needs. Robert presented with decreased muscle strength, impaired balance, and limited endurance, all of which significantly impacted his quality of life and increased his dependence on assistance for routine tasks.

An assessment was created based on Robert’s needs and it was time to get started. Robert was more than ready to get therapy started and get back on the road to recovery. Robert required moderate assistance for transfers and moderate assistance with bed mobility, and was only able to ambulate 10ft with supervision.

Over the next month and a half, Robert worked hard with his therapy team. While talking to Robert, he stated, “I came here and was not sure what the results would be. I just knew I had to give my best.” Robert was ready to tackle therapy each day and would be up and ready for each session.
Robert pushed himself hard to regain his strength and looked forward to a discharge date; he stated, “That is what kept me motivated each day.” Robert is independent with bed mobility and transfers and is now able to ambulate 150 feet.

Robert was discharged to an assisted living facility, but before leaving, he stated, “They did very well, the entire team knew what they were doing, and they set me up for success.” We wish him the best on his journey and hopes of knowing he always has friends to visit at Gallatin Center.
